In order to make the most of our time together, please print and fill-out our new patient paperwork before your first appointment: patient history (adult or child), financial policy, and the neck or low back forms as they apply (see below). Because you’ll be moving around, please bring or wear athletic clothes (shorts and a t-shirt, etc.).

Expect your first visit to last approximately one hour, depending on the complexity of your condition. Follow-up visits are scheduled in 30 minute increments.

At your first visit, you can expect Dr. Mennerick to review your history with you and perform a detailed exam which may include orthopedic tests, functional screenings to watch how you move, gait analysis, neurological testing, muscle strength testing, soft tissue assessment, and other exams as needed. Although not necessary on all patients, imaging studies may be ordered to help with determining the extent of the condition. Lab tests are helpful in detecting underlying problems and can be performed in-office. Once all the findings have been gathered, the doctor will give you a detailed explanation of your condition and the decision will be made whether or not she can help you with your problem or if you would be better served with a referral to another provider. If you and Dr. Mennerick decided to proceed with care, treatment recommendations will be made and begins on the first visit.

Insurance:

Please call our office for more guidance on insurance, billing, and payments. We are “In-network” with BlueCross BlueShield PPO and Blue ChoiceSelect (ID numbers start with XOU) networks. As a courteous, we will submit claims to other insurance companies; however, we are considered “Out-of-Network” with all other insurances, and payment is expected at the time of service. Because of Dr. Mennerick’s treatment approach that utilizes exercises and interventions to your daily activities, the goal is to release you from care as soon as possible to minimize costs.
